Please fill separate forms for each participant if they are participating individually. For example Jenny and John live in the same house and they are siblings.  Jenny will fill one form and select Voice English and John will fill another form and select Dance category

One participant wants to participate in more than one category. Please fill one form with participant first name and last name. Select the categories the participant wants to participate in.  

Please fill one form as they are participating in group. For example Jenny and John live in the same house and they are siblings. They want to do a group dance.  Jenny will fill one form and select Dance, choose number of participants as 2 and mention Johns name that will be asked when payment is complete.

Please fill one form if you are participating in a group perforce. The primary contact will mention the name on registration form. All other names can be provided after the payment is complete.
